Community Hands Advancing Maternal Health Promotion (CHAMPions)

CHAMPions is a collaboration between Driscoll Children's Hospital, Driscoll Health Plan, the Driscoll and Texas A&M Health Global Institute for Hispanic Health, and the Texas A&M College of Nursing. We are piloting a FREE educational home visiting program delivered by specialized Community Health Workers to support pregnant women, postpartum mothers, and their families.

  • Family wellness checks
  • Prenatal and postpartum guidance
  • Personalized care plans
  • Referrals to local providers and services
  • Free mental health services
  • Emotional well-being support during and after pregnancy

  • Pregnant OR had a baby in the last 9 months
  • Living in service counties
